The approaches to triage in a field hospital are presented based on the experience of 20 years long activity of the Field multipurpose hospital of All-Russian Center for Disaster Medicine “Zaschita”. 4. Goncharov S.F., Bystrov M.V., Kudryavtsev B.P., Savvin Yu.N. Clinical Recommendations on Disaster Medicine (Delivery of Medical Care to Casualties of Emergency Situations)

A piece of work is presented on preparation of clinical recommendations on disaster medicine. It is stressed that the recommendations are being elaborated with due consideration of the principles of evidence based medicine and are the result of cooperation of a big number of specialists. The content and the purpose of the recommendations are discussed. It is noted that 7 recommendations were prepared and approved in 2013 and 11 were prepared in 2014. Special attention was paid to introduction of approved recommendations into practice.

6. Brizhan’L.K., Davydov D.V., Kerimov A.A., Arbuzova Yu.V., Chirva Yu.V. Use of New Rod Apparatus for Treatment of Damages of Bones of Extremities and Pelvis

The experience is integrated of use of the new rod apparatus (military field rod kit) in 43 victims. In all the cases of treatment «damage control» tactics and stepwise osteosynthesis of bone fragments were used. The rod apparatus was used at the first stage of treatment. The possibility of reposition of bone fragments, the timing of the apparatus use, its average mass, x-ray transparency as well as versatility of the construction were studied. The possibility is demonstrated of fast, simple, efficient external fixation of fragments of damaged long bones of extremities and of pelvis bones.

Odinak M.M., Vorob’yov S.V., Emelin A.Yu., Emel’yanov A.Yu. Comprehensive Assessment of Neurological and Neuropsychological Changes in Patients with Post-Traumatic Cognitive Impairments

The results are presented of a comprehensive clinical and neuropsychological examination of patients with cognitive impairments after craniocerebral traumas of different severity.

It is noted that the patients complained more often of impaired concentration and intellectual work capacity. In neurological status of those examined cerebellar disorders and disruption in motional sphere. It is found that there prevailed disorders of dynamic and regulatory functions.

Mitin I.N. Coping of Drivers and Road-Traffic Safety

The necessity of inclusion of psychophysiological tests into medical examinations of professional drivers to assess their coping is substantiated. The analysis is done of the authors’ assessment of coping of different categories of drivers against their accident incidence rate. The ways of perfection of medical support of road traffic safety by detection of drivers with poor prognosis of accident-free work are suggested.

The issue of information support of preparation and carrying out of medical evacuation is discussed. The medical evacuation form “Epicrisis for medical evacuation” elaborated in Territorial center for disaster medicine of Moscow Region is analyzed. It is noted that the use of this document allowed optimizing preparation of patients for transportation, organizing medical evacuation and so on.

11. Baranova N.N. Expert Assessment of Program of Additional Professional Training of Specialists of Aeromedical Teams. 

Report 1

The expert assessment of program of additional professional training of specialists of aeromedical teams is presented. The contingent of experts is characterized. It is noted that a questionnaire for 201 specialists of territorial centers for disaster medicine and of emergency consultative medical care and medical evacuation wards was elaborated to promote an expert assessment of the content and structure of the program “Sanitary aviation evacuation of sick and wounded”.

Inquiry returns of experts to 8 questions out of 17 are analyzed in Report 1.

INTERNATIONAL COOPERATION

12. Goncharov S.F., Kipor G.V. International Cooperation in Field of Disaster Medicine

The main directions and results of the international cooperation of All-Russian Center for Disaster Medicine “Zaschita” in 2014 are presented. It is noted that the Center was acting as an accredited WHO Collaborating Center for the coming period in the area of preparedness securing for emergency situations.

Some suggestions are formulated for WHO program in the sphere of enhancement of national potential of control in health service sphere in case of emergency situations and natural calamities and stability of health service system.
