Summary. The objective of the study is to formulate scientifically substantiated proposals for the Road Safety Strategy in the Russian Federation for 2025–2030 in terms of implementing measures aimed at further improving road safety and reducing mortality from road traffic injuries (RTI) in the special conditions of the Far North and the Arctic zone of the Russian Federation.

Materials and methods of the study. Using the comparative analysis method, a review of federal and regional regulatory documents was conducted, the subject of which was the issues of ensuring RTI, strategic development of the northern territories of the country, as well as the results of domestic and foreign scientific research, the subject of which was the regional features of RTI in the Far North and the Arctic zone and organizational approaches to providing first aid and medical care to victims of RTIs. The search for scientific literature was performed in the eLibrary, PubMed, Scopus systems. The research articles published between 2005 and 2025 were selected for the analysis.

Research results and their analysis. It is advisable to include the following set of organizational measures in the new Strategy for 2025–2030:

– Ensure the operation of the Unified Interregional Information System for Monitoring the Medical and Sanitary Consequences of Road Accidents.

– Develop interregional schemes for medical evacuation of road accident victims by various modes of transport to the nearest medical organization to the accident site that performs the functions of level I, II or III trauma centers, taking into account the severity of the victim’s condition.

– Organize advanced training for physicians and specialists with secondary medical education of the subjects on issues of providing emergency medical care and disaster medicine; on issues of providing emergency medical care to victims with multiple and combined injuries (polytrauma), including those in a state of hypovolemic shock; an algorithm of actions in the event of simultaneous admission of two or more victims in serious condition, the threat of contamination with hazardous or other toxic substances; in the event of other situations with possible concomitant cold damage to victims.

– In regional schemes for routing victims in road accidents who are in a serious condition, provide for priority medical evacuation, carried out by various types of transport, directly to medical organizations performing the functions of trauma centers of levels I or II, with a preliminary assessment of the actually possible volume of medical care in the organization in each specific case.

– In a medical organization that functions as a level III trauma center, limit the scope of medical care to urgent measures to stabilize victims in a severe condition and prompt medical evacuation to a higher medical organization, etc.
