1. Zemlyannikov D.A., Avetisov G.M. Experience of Team Building Exercises of Service for Disaster Medicine of Ministry of Health of Russia and of Defense Ministry’s Service for Disaster Medicine in Liquidation of Medical and Sanitary Consequences of Accidents with Nuclear Materials

The results are discussed of exercises of Service for Disaster Medicine of Ministry of Health of Russia and of Service for Disaster Medicine of Defense Ministry in issues of medical support in accidents with nuclear materials. It is noted that the exercises made evident the need for changes in some major provisions of Defense Ministry’s regulation documents on liquidation of consequences of accidents with nuclear materials. A conclusion is drawn that All Russian Service for Disaster Medicine should be ready not only for liquidation of consequences of radiation accidents at nuclear power plants but of possible accidents with nuclear materials.

The structure and the main directions of activity of Voronezh Oblast Clinical Center for Disaster Medicine are presented. The special attention is paid to work on perfection of organization of medical care delivery to victims of road traffic accidents and to development of sanitary aviation in the region.

3. Timkin V.A., Zainul’dinova V.A., Ripp O.G. Concept of Development of First Aid in Buryat Republic and Possibility of its Realization in other Subjects of Russian Federation 

The suggestions are presented on perfection of assessment of activity of first aid services to enhance first aid delivery in the territory of the Russian Federation within the framework of realization of the State program of health service development till 2020. A suggestion is made on introduction of an integrated index including the assessment of state of material and technical base and of personnel of first-aid service as well as of indicators of its response speed, its availability and quality in urgent and emergency forms at prehospital and hospital stages and at medical evacuation stage.

The results of methodological support of first-aid wards in organization of on-site medical care in case of road traffic accidents and emergency situations. The suggestions are made on perfection of the activity in the sphere.

CLINICAL ASPECTS OF DISASTER MEDICINE

6. Baksanov Kh.D., Zhigunov A.K., Miziev I.A., Kardanova L.D., Berov R.B., Dabagov O.Yu., Shidugov A.Z., Kardanova M.F. Management Tactics for Concomitant Craniocerebral and Orthopedic Traumas

The experience of treatment of 336 casualties with craniocerebral traumas of different severity combined with orthopedic injuries is presented. The case management and fracture fixation methods are discussed in detail.

The conclusion is drawn that the use of primary and early osteosynthesis in casualties with concomitant cranioorthopedic trauma leads to better treatment results, lower mortality and faster social rehabilitation of victims.

The experience is presented of organization and carrying out of dosimetric investigation of patients exposed to radiation as a result of a radiation accident or incident admitted to the specialized clinic of the A.I.Burnazyan Federal Medical Biophysical Center.

8. Budkevich L.I., Rozinov V.M., Bezuglova T.V., Zaitseva T.V., Shilkin I.P., Dolotova D.D., Astamirova T.S. Vacuum Therapy Efficiency in Treatment of Children with Soft-Tissue Wounds

The efficiency of vacuum therapy in treatment of children with soft-tissue wounds is presented. Fifty children with wounds of different ethiology were examined. In 50% of the cases vacuum therapy was used and modern wound coverings in the rest of the cases (comparison group). The efficiency of methods was estimated according to the results of immunohistochemical, microbiological, and photoplanimetric investigations. It is shown that vacuum therapy creates favorable conditions for wound process course so that the difference is statistically significant in comparison with the other group.

An analysis of the experience of use of sanitary aviation in liquidation of medical and sanitary consequences of road traffic accidents along the Ring road of St. Petersburg is presented. A conclusion is drawn that the key advantage of use of sanitary aviation in first-aid delivery to road traffic accident victims is in significantly faster medical evacuation.

The role of public private partnership in development of sanitary aviation in St. Petersburg and Leningrad Oblast within the framework of realization of the pilot project “Development of Sanitary Aviation in Subject of the Russian Federation in 2014-2015” is analyzed. The ways of optimization of expenses and possible patterns of public private partnership in the sphere are discussed. A suggestion is made on creation of sanitary aviation centers in Federal Districts. The main functions of such centers are defined.

The analysis is presented of the results of tactical special training exercises on actions of leadership of Health Department of Moscow, of medical organizations and city’s operation services after detecting a case suspicious of highly contagious viral hemorrhagic fever – an infectious disease demanding measures of sanitary protection of city’s territory.

A review of literature on issues of organization of medical care delivery to casualties of road traffic accidents with polytrauma is presented. The demands are discussed to trauma centers of the 1st, 2nd and 3rd levels. The experience of activity of trauma centers of St. Petersburg is analyzed. A conclusion is drawn on expediency of setting up in country’s regions and major cities of system of trauma centers within the framework of solving problems of disaster medicine and medical support of population in emergency situations.

The experience of liquidation of medical and sanitary consequences of a road traffic accident in Neryunga Region of Republic of Sakha (Yakutia) is presented. The activity of teams taking part in on spot medical care delivery to road traffic accident casualties and in medical evacuation is analyzed. A number of organizational problems demanding decision taking on Federal level is noted.
